Malar rash and SCLE lesions of the ears. This is systemic lupus erythematosus with high ANA and high DSDNA of 1:1280. DSDNA is the more specific test for lupus and such patients have a higher risk of lupus nephritis or kidney involvement. . Systemic lupus can have wide ranging symptoms and severe complications. . This patient presented with a rash and swoillen ankles, and was found to have the following: . Fatigue Sun sensitivity Malar rash of cheeks which can be seen here Oral ulcers SCLE lesions of the ears Anemia requiring a transfusion (low red blood cells) Leukopenia ( low white blood cells) Thrombocytopenia ( low platelets) Hypocomplementemia (low complements) Neurologic symptoms of confusion Arthritis (joint pain) Serositis (shortness of breath and found to have a pleural effusion) Enteritis (abdominal pain) Nephritis with hematuria and proteinuria (kidney inflammation) . Part 2: it was the perfect storm. He was hypertensive and tachycardic. A small dose of IV beta blocker should have smoothened him out but here we were: now with a severely low blood pressure and a heart that continued to fibrillate at high rates. Pressure filled the room and weighed me down. Fix it - fix him, I thought to myself. "Push a 500 bolus of normosol under pressure," I said quickly. He was a dialysis patient and I hated to give him fluids but the bolus would either buy me some time or fix him. A steady stream of fluid flowed into his IV tubing and into his arm. His mean arterial pressure (MAP) put my stomach in a knot. Still no response from my senior as I checked my phone. His nurse came around the room and began to say, "his MAPs aren't that much bett-" before I shouted for a crash cart. "Do you want me to call a code?" Someone asked. "No." I responded staring at his monitors. I remember asking myself what was holding me back before I decided to move forward with my own plan. Was it my angry fellow? Was it the disapproval of my senior? Was it my attending who thought I went rogue? It didn't matter. "Let's hang another 500 of normosol and get some pressors on board. Someone pull levophed and lets titrate until we have a MAP of 50-55. I want the amiodarone drop off until his pressures are back up. I'll order a formal EKG and a full set of labs. We'll go from there.... " -m

circa summer 2017. it was a fun day, that day, unbeknownst to me at that time, the beginning of many chapters in my life. this was the summer of many many lessons. most importantly, I learned to embrace my noise in addition to my ambitions . I decided that I would lead a healthier and noisier life. “needles + noise” was born, a digital space where women are encouraged to lead healthier and noisier lives despite their many ambitions . two years later, I feel oh so compelled to pick up where I left. here’s to leading healthier + noisier lives 🥂.

The most traumatizing experience I had was when I witnessed my first patient death. . It was Monday May 14, 2018 at the Winchester ED and we got a call that a triple trauma was coming in. . It was an MVA involving a 4 year old female who was unrestrained, two males - 10 and 12 years old, and their mother, who was driving. . The girl had a thready pulse, a GCS of 3 and was intubated at the scene. She had been thrown of the window. . It was a huge trauma room, enough space for 20 of us. I remember standing in a corner, horrified, trying to stay out of the way as a student. . The little girl coded 2 times prior to her head CT but made it back to the room alive. We got the results back - showing a skull fracture and large subdural hematoma. . She was immediately ordered to be flown to a level 1 trauma center, but it was raining and the helicopters were grounded. . She kept coding and the team of doctors, nurses and techs did everything to keep her alive. . After several unsuccessful attempts, the doctor running the code had to make the very difficult call to stop because she was clearly brain dead. . I started crying uncontrollably. Flew out of the trauma room and locked myself in the bathroom. I couldn’t understand why or how this could happen to a girl so small and innocent. . When I finally got out, I bumped into one of the PAs I had previously worked with who was very worried about me. . I told him I was fine. I wasn’t. Tears streamed down my face and I couldn’t describe what I was feeling. I felt guilty for crying because I felt that I had no right. That girl had a family who was distraught over her loss - so who was I? . It took me a long time to heal. But I will never forget that little girl or that experience. . I’m sharing this story because I know many of you have or will experience loss on the job. Please don’t keep it in. Talk about it with the people who experienced it with you. Talk about it with your mentors. Talk about it and work through the emotions. . I pray for the little girl and her family and I hope they find some semblance of peace. . I was inspired by @cookingforboards  (who is amazing btw) to write about my relationship to someone who is not in medicine. If you’re going to date someone in general, you need to find someone who will support you in whatever you want to do and will understand that your life is multifaceted and can be hectic. I’m lucky enough to have an amazing partner who is more accepting of my newly hectic work/school life than I am. He’s always gone out of his way when I needed him. Like the other day when I witness a patient’s imminent death: he met me before he had to go to work so that I could hug him and talk to him about it. He dropped me off at work last night and picked me up this morning, and then stayed over for awhile until I passed out to keep me company. Sometimes he shows up with my favorite tea or a small dessert. He is also understanding of my current financial situation and pays for most of our date nights. I’m definitely lucky. And he reminds me of why it’s so important never to settle. Find the person who will be there for you no matter what and who makes you happy. If we’re being real, if you’re going into a demanding job, you ain’t got time to mess around and play games or be unhappy. And most importantly don’t forget that relationships go both ways whether you’re working crazy hours or not. You cannot act like your problems are more important bc you’re in school, because they’re not. My partners issues are mine no matter how big or small. Be there for them too. Do the little things. The little things are what add up, not infrequent grand gestures. Set a night every week to go in a date. It also gives you something to look forward to. Bring them their favorite drinks and cook for them. It doesn’t really matter. What matters is that you put in the effort you receive from a supportive partner.

Today my Ecuadorian host family (from when I stayed here in college!) took me to Quilotoa Crater, located around 3 hours away from Quito. Hiking down takes 30 minutes while hiking up takes an hour and a half. The “road” is covered in sand and pebbles and prone to a lot of slippage. I almost wiped out 100 times. My stomach felt better this morning but started hurting again towards the end of the trip so going to try to get some rest tonight. I’m definitely thankful to be with family while I’m not feeling so good!! 🇪🇨 Hoy mi familia anfitriona ecuatoriana (de la universidad) me llevó al cráter Quilotoa, ubicado a unas 3 horas de Quito. Caminar hacia abajo toma 30 minutos, mientras que una caminata hacia arriba toma una hora y media. El “camino” está cubierto de arena y guijarros y es propenso a muchos deslizamientos. Casi resbalé 100 veces. I used to almost exclusively think of myself as a failure. It was hard not to in the competitive pre-med environment at my undergrad. I graduated with a 2.55 sGPA and a 2.98 cGPA. I still wanted to become a doctor, but I felt so dejected by my numbers and by watching my classmates' successes. I let myself feel this way for most of the latter half of my time in college. I sought advice and inspiration, but my efforts were in vain. So, I took it upon myself to develop a plan and strategy to position myself best for a med school application process. I researched my options nonstop and learned everything I could about post-baccs, masters programs, and what it takes to be a competitive applicant. Then I set my plan in motion. • That was 3 years ago. Since then I've graduated from a 2 year MS Bio program, a 1 year MPH program (both of which I got a 4.0 GPA in), taken the MCAT, applied to med school, and been accepted to multiple schools. It wasn't easy. I cannot stress that enough. I worked harder than I ever have to get to where I am today to make myself a competitive applicant. It took years of strategizing, of tears, of grit and most importantly, of dedication. I was and still very much am dedicated to my future career as a physician. • I no longer think of my time in undergrad as a failure. It sucked to go through that, don't get me wrong. But, I've learned so much from that experience and grown as a person more than I would have without having been on this journey. We're more than our numbers as pre-meds. Remind yourself of "your why", surround yourself with support, and persevere. Time to defeat the buns!! . . How many times have you told a patient or thought in a room with patient you should be moving your body more! Even if its parking your car further away or taking the stairs instead of the elevator. Working out is one of the most under utilized anti depressants and source of energy! Sleep has been an increasingly rare thing to come by and that 20/30 minutes of taking the time to exercise makes ALL the difference! Itll make you feel human at the end of a very rough day or start your day off on the right foot! And I find the funner the workout clothes the more I want to put them on 😍 no matter how tired or frustrated I am. Find the time that works for you and make taking care of you a priority!! . . Quick side note: I'm sure you're wondering what all the braces are. The month before match might be one of the hardest times for me mentally. I was a stressful mess. And I thought working out would be a fabulous way to relieve that stress. And in a lot of ways I was right! ...but I worked my body too hard and reactivated old injuries. And I'm still paying the price for it. So here's the reminder to be compassionate with yourself and listen to your body!! Find the workout that motivates you to workout and makes you feel good, but dont find yourself so deep in a hole that you try to use exercise to dig yourself out. That's how you get hurt ❤ love yourself and your body!! Everyone knows we spend enough time telling our patients that. We should take a chance and listen 🤗🤗
